POTS / Dysautonomia Physical Therapy

Postural Orthostatic Tachycardia Syndrome (POTS) is a form of autonomic dysfunction where the body has difficulty regulating heart rate and blood pressure—especially with changes in posture. Many people experience dizziness, rapid heart rate, fatigue, brain fog, nausea, temperature regulation issues, shortness of breath, and exercise intolerance. While POTS can feel overwhelming, the right physical therapy program can significantly improve function, reduce symptoms, and help you gain confidence in your daily activities.

What a POTS Physical Therapy Program Looks Like

Evaluation:

A comprehensive assessment includes:

  • Heart rate and blood pressure changes with position
  • Breathing mechanics
  • Strength, posture, and mobility
  • Activity triggers and daily patterns
  • Co-existing conditions (EDS, MCAS, migraines, GI issues, anxiety, chronic pain, or deconditioning)

Recumbent and supine exercise training

PT begins with exercises done in gravity-minimized positions such as:

  • Recumbent bike

  • Rowing machine

  • Supine core work

  • Side-lying leg strengthening

  • Pilates-inspired breathing and stabilization

These exercises help build cardiovascular endurance without provoking tachycardia.

Graded Exposure To Upright Activity

As tolerance improves, PT gradually introduces:

  • Seated-to-standing drills

  • Standing balance work

  • Modified squats and lunges

  • Short upright cardio intervals

  • Functional activities like stairs and carrying tasks

The goal is to retrain the autonomic system without overwhelming it.

Breathing Training & Nervous System Regulation

Many individuals with POTS have dysfunctional breathing patterns that worsen dizziness and tachycardia. PT incorporates:

  • Diaphragmatic breathing

  • Rib mobility

  • Vagus nerve–supportive strategies
